Registration

The Chrysalis Board is pleased to present a new and improved registration form! The new registration form has 3 options for completion:

1. Print, Complete (Handwritten), and Mail -- This option is the same as the old form. You may choose to print the form and complete with handwritten responses and then mail to the appropriate registrar.
2. Complete Electronically, Print, and Mail -- This option will allow you to complete the form by typing in the form. Once you have completed the form choose the print form button and then mail to the completed application to the appropriate registrar.
3. Complete Electronically and Submit via e-mail! -- This option allows you to submit your application electronically by filling out the form on your computer, saving the document, and then email it.

The email option is good for both Boy's and Girl's applications and/or questions concerning registration.

PLEASE NOTE: 3 separate forms are needed - an application, sponsor form & parent permission form.
