Chrysalis C.A.R.E.S. Application Instructions

The Chrysalis Flight / Journey is open to Christians of any denomination who are seeking a closer relationship with the Lord.

Do You Have a Sponsor?

In order to attend a Chrysalis Flight / Journey, you must have a sponsor - someone who has attended a previous Walk / Flight and will serve as your guide before and after the weekend. If you do not know anyone who could serve as your sponsor, please contact us.

Cost

The cost of attending the Chrysalis Flight / Journey is $65; this covers all food, accommodations, supplies, etc. used during the weekend. Scholarships are available for those who are in need. Please let your sponsor know if you are in need of a scholarship, and the fee will be covered for you.

Submitting an Application (offered as Word or as a PDF)

Please try to submit applications two weeks before the flight so we may plan appropriately.

There are 2 parts to the application:

1. First download both pages of the forms by clicking on either of these links:
   Part 1 and 2 as an Adobe document
   Part 1 and 2 as a Word document
2. Complete Part 1 with your own personal information, answering all questions.
3. Please give the following to your sponsor:
   a. Part 1 of the application is to be completed by you and your Parent/Guardian.
   b. Part 2 of the application, which will be completed by your sponsor (you should leave it blank).
4. Your sponsor will complete Part 2.
5. After the Applications are complete, you can chose to Mail them to the Address found on the Application or Scan and Email them to Chrysalisapplications@gmail.com
6. Please make checks payable to the Chrysalis C.A.R.E.S. This can be mailed with the Application or Paid at Registration on Day One of your weekend… (The $65 will be due at the time of your Chrysalis weekend).
